How the generator decides

It does not. The hazards and controls are yours, from the risk assessment. The generator lays them out so each step of the work says which controls apply, and prints the result in the order a site manager reads it: scope, people and plant, hazards, sequence, emergencies, sign off, briefing record.

Common questions

Where do the hazards and controls come from?
From your risk assessment. If it is in the register on this site, pick it on the first step and the hazards and the controls ticked as in place come across. Otherwise type them. The generator never invents controls for a job it has not seen.
Is the finished document HSE approved?
No document is HSE approved. HSE does not approve method statements. The output follows the structure principal contractors expect, and the contractor must check it against the actual site and job before work starts.
Do I need the risk assessment first?
Yes, in practice. A method statement puts the controls from the risk assessment into the order of work. Without the risk assessment there is nothing to put in. Do the risk assessment on this site and it is one click away.
